Responsibilities
- Develop and implement individualized case management plans for 25+ clients facing homelessness, domestic violence, or substance abuse challenges
- Coordinate with healthcare providers, legal services, and community partners to create holistic support networks
- Conduct comprehensive needs assessments using evidence-based screening tools
- Document client interactions and outcomes in compliance with HIPAA and organizational standards
- Provide crisis intervention and emotional support during critical life transitions
- Lead community outreach initiatives to expand service accessibility
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- 3+ years of direct case management experience with vulnerable populations
- Licensure in Illinois (LCSW or LCPC required)
- Proficiency in electronic health records (Epic or similar systems)
- Deep understanding of trauma-informed care principles
- Certification in Crisis Intervention or Motivational Interviewing
- Valid Illinois driver's license with clean driving record
