Responsibilities

- Conduct comprehensive needs assessments and risk evaluations
- Provide crisis intervention and trauma-informed counseling
- Maintain accurate case documentation and comply with regulatory requirements
- Lead outreach initiatives to identify and engage at-risk community members
- Collaborate with interdisciplinary teams to address systemic barriers
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- 3+ years of direct case management experience in social services
- Licensure in New York (LMSW/LCSW required)
- Expertise in trauma-informed care and crisis intervention protocols
- Strong advocacy skills with experience navigating social service systems
- Ability to work effectively with diverse populations and cultural competency
- Proficiency in electronic health records and case management software
