Responsibilities
- Develop and implement comprehensive case management plans for 25+ clients facing housing instability, mental health challenges, or economic hardship
- Conduct biopsychosocial assessments and coordinate care with healthcare providers, legal advocates, and community organizations
- Provide crisis intervention and trauma-informed support during emergencies (domestic violence, homelessness, etc.)
- Lead outreach initiatives to connect underserved populations with essential resources (SNAP, Medicaid, job training)
- Maintain meticulous documentation in compliance with HIPAA and funder requirements using Salesforce CRM
- Mentor junior staff through case consultations and skill-building workshops
- Analyze program data to identify systemic barriers and recommend policy improvements
Qualifications
- Master's degree in Social Work, Counseling Psychology, or related field (LCSW/LPC preferred)
- 5+ years direct experience with vulnerable populations in urban settings
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Expertise in navigating Illinois social service systems (DCFS, IDHS, etc.)
- Advanced proficiency in electronic health records and data analysis tools
- Bilingual Spanish/English proficiency required
- Valid Illinois driver's license with reliable transportation for home visits
- Proven success in securing grants and managing program budgets
