Responsibilities
- Assess client needs and develop comprehensive case management plans using evidence-based practices
- Coordinate interdisciplinary services including housing placement, mental health support, and vocational training
- Conduct crisis intervention and safety planning for high-risk individuals and families
- Maintain detailed client documentation and ensure compliance with federal/state regulations
- Facilitate support groups and community workshops on life skills and self-advocacy
- Collaborate with community partners to expand resource access and service gaps
- Train junior staff on trauma-informed care methodologies and ethical practices
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or related field required
- Minimum 3 years' experience in direct client services with vulnerable populations
- Licensure in Illinois (LCSW, LCPC, or equivalent) or eligible for immediate licensure
- Advanced knowledge of community resources, housing vouchers, and public assistance programs
- Crisis intervention certification and experience with trauma-informed care models
- Exceptional cultural competence and ability to work with diverse populations
- Strong data management skills and proficiency in electronic health records (EHR) systems
- Valid Illinois driver's license and reliable transportation for home visits
