Responsibilities
- Develop and implement individualized service plans for clients facing homelessness, mental health challenges, and economic instability
- Conduct comprehensive assessments using standardized tools to identify needs, strengths, and barriers
- Coordinate multidisciplinary care teams including therapists, housing specialists, and legal advocates
- Provide crisis intervention and safety planning for high-risk situations
- Advocate for systemic change through policy engagement and community coalitions
- Maintain accurate documentation compliant with federal/state regulations
- Facilitate psychoeducational groups on life skills, financial literacy, and emotional regulation
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- 3+ years direct case management experience with vulnerable populations
- Licensure as LCSW in Illinois or eligibility for immediate supervision
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Expertise in navigating HUD/HHS systems and benefits enrollment
- Proven ability to build rapport across diverse cultural and socioeconomic backgrounds
- Advanced data analysis skills for outcome measurement and reporting
- CPR/First Aid certification or willingness to obtain within 30 days
