Responsibilities

  • Client Assessment & Planning: Conduct comprehensive biopsychosocial assessments to develop individualized service plans that address the unique needs of each client.
  • Resource Navigation: Act as a primary advocate, connecting clients with essential resources including housing, healthcare, employment, and financial assistance.
  • Crisis Intervention: Provide timely support during acute crises, ensuring client safety and facilitating appropriate referrals to emergency services when necessary.
  • Documentation: Maintain accurate, timely, and confidential case notes and electronic health records in compliance with state regulations and agency policies.

Qualifications

  • Education: Bachelor’s degree in Social Work (BSW), Psychology, or a related field required; Master’s degree (MSW) preferred.
  • Licensure: Current LCSW or LCSA license in the State of California is highly desirable.
  • Experience: Minimum of 3-5 years of progressive experience in case management, social work, or a related human services field.
