Responsibilities
- Develop and implement comprehensive case management plans for diverse client populations
- Coordinate with healthcare providers, legal services, and housing agencies to ensure holistic support
- Conduct risk assessments and crisis interventions for high-need individuals
- Maintain detailed documentation and compliance with state/federal regulations
- Lead outreach initiatives to connect marginalized communities with available services
- Train and mentor junior case staff through evidence-based practices
- Analyze community needs to inform program development and policy recommendations
Qualifications
- Master's degree in Social Work, Psychology, or related field
- 3+ years direct case management experience with vulnerable populations
- Licensure as LSWAIC or equivalent in Washington State
- Proficiency in electronic health records (EHR) systems and data analysis
- Demonstrated cultural competency and trauma-informed care expertise
- Strong crisis intervention and conflict resolution skills
- Valid Washington State driver's license and reliable transportation
