Responsibilities
- Develop and implement individualized care plans for complex cases involving at-risk youth and families
- Conduct comprehensive psychosocial assessments and coordinate multi-disciplinary interventions
- Provide crisis intervention and safety planning for clients experiencing acute distress
- Collaborate with community partners to build sustainable support networks
- Train and mentor junior case management staff on best practices
- Monitor program outcomes and contribute to continuous quality improvement
- Advocate for policy changes addressing systemic barriers to client success
Qualifications
- Master's degree in Social Work, Counseling, or related field required
- Minimum 5 years progressive case management experience in social services
- Licensure (LICSW, LMHC, or equivalent) in Washington State
- Expertise in trauma-informed care and crisis intervention protocols
- Proven ability to navigate complex community resources and systems
- Advanced data management and outcome reporting experience
- Strong cultural competency with diverse populations
- Valid Washington State driver's license and reliable transportation
