Responsibilities

  • Develop and implement comprehensive case management plans for 25+ clients facing homelessness, mental health challenges, and economic instability
  • Facilitate multi-agency collaborations with healthcare providers, housing authorities, and legal advocates
  • Conduct biopsychosocial assessments using evidence-based frameworks

Qualifications
