Responsibilities
- Develop and implement comprehensive case management plans for 25+ clients facing homelessness, mental health challenges, and economic instability
- Facilitate multi-agency collaborations with healthcare providers, housing authorities, and legal advocates
- Conduct biopsychosocial assessments using evidence-based frameworks
- Provide crisis intervention and safety planning for high-risk individuals
- Oversee team of 3 junior case managers through mentorship and quality assurance reviews
- Analyze client outcome data to refine service delivery models
- Advocate for policy improvements at community forums and legislative hearings
Qualifications
- Master's degree in Social Work, Counseling, or related field (MSW preferred)
- Minimum 5 years progressive case management experience in social services
- Licensure in Washington (LSW, LMHC, or equivalent)
- Expertise in trauma-informed care and harm reduction principles
- Advanced knowledge of HUD housing programs and Medicaid systems
- Proven ability to manage complex caseloads with competing priorities
- Experience supervising staff or interns in clinical settings
- Proficiency in electronic health records (Epic or Cerner preferred)
