Responsibilities
- Develop and implement individualized service plans for clients facing homelessness, mental health challenges, or economic hardship
- Conduct comprehensive psychosocial assessments and risk evaluations
- Coordinate with healthcare providers, legal services, and housing agencies to ensure holistic support
- Advocate for clients during housing court proceedings and benefit application processes
- Provide crisis intervention and trauma-informed counseling during critical life transitions
- Maintain detailed case documentation while maintaining HIPAA compliance
- Mentor junior staff through supervision sessions and peer training workshops
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- 3+ years direct case management experience with vulnerable populations
- Licensure in NY (LMSW/LCSW) or eligibility for immediate certification
- Proven success in navigating complex social service systems (SNAP, SSI, Section 8)
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Advanced proficiency in electronic health records (EHR) systems
- Valid NY State driver's license with clean driving record
