Responsibilities
- Develop and implement individualized service plans for clients facing housing instability, mental health challenges, or economic hardship
- Conduct comprehensive needs assessments and coordinate multidisciplinary support services
- Provide crisis intervention and trauma-informed counseling to vulnerable populations
- Build and maintain partnerships with community organizations, healthcare providers, and government agencies
- Monitor program outcomes through data collection and reporting to funders
- Mentor junior staff and supervise case management interns
- Advocate for policy changes addressing systemic barriers to social services
Qualifications
- Master's degree in Social Work, Counseling, or related field (LCSW/LPC preferred)
- Minimum 4 years of direct case management experience in social services
- Proven expertise in crisis intervention and trauma-informed care
- Strong knowledge of Chicago's social service ecosystem and resource networks
- Excellent documentation skills with proficiency in case management software
- Certification in Mental Health First Aid or equivalent
- Ability to work flexible hours including evenings and weekends
- Bilingual proficiency in Spanish highly desirable
