Responsibilities
- Develop and implement individualized service plans for 25+ clients across diverse demographics
- Coordinate multi-agency collaborations with healthcare providers, schools, and government entities
- Conduct comprehensive psychosocial assessments and risk evaluations
- Lead crisis intervention protocols for high-risk situations including domestic violence and homelessness
- Utilize our proprietary care management software for documentation and outcome tracking
- Train and mentor junior case staff in trauma-informed methodologies
- Analyze community needs data to inform program development and advocacy initiatives
Qualifications
- Master's degree in Social Work (MSW) with LCSW/LISW licensure in Illinois
- Minimum 5 years experience in direct service with vulnerable populations
- Expertise in trauma-informed care and crisis intervention techniques
- Proficiency with electronic health records (EHR) systems and data analytics
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
- Demonstrated success in securing external funding and grant management
- Fluency in Spanish or another relevant neighborhood language
- Valid Illinois driver's license with reliable transportation
