Responsibilities
- Develop and implement individualized case management plans for at-risk populations
- Conduct comprehensive psychosocial assessments and coordinate multi-agency interventions
- Oversee crisis response protocols for domestic violence and homelessness cases
- Maintain compliance with HIPAA and state regulatory requirements
- Train junior staff on trauma-informed care methodologies
- Analyze program metrics to identify service gaps and improvement opportunities
- Collaborate with healthcare providers, schools, and government agencies
Qualifications
- Master's degree in Social Work, Psychology, or related field
- 3+ years of direct case management experience in urban settings
- Licensure as LCSW or LCPC in Illinois
- Certification in Trauma-Focused Cognitive Behavioral Therapy
- Proficiency with electronic health record systems (Epic preferred)
- Demonstrated crisis intervention skills with complex cases
- Experience writing successful grant proposals and program reports
- Valid Illinois driver's license with reliable transportation
