Responsibilities
- Client Advocacy: Serve as the primary point of contact for clients, advocating for their rights and ensuring access to necessary social services and resources.
- Case Planning: Develop, implement, and monitor individualized case plans that address the physical, emotional, and social needs of clients.
- Crisis Intervention: Provide immediate support and intervention during mental health or family crises to ensure safety and stability.
- Documentation: Maintain accurate and timely electronic health records (EHR) and case notes in compliance with agency and regulatory standards.
- Community Collaboration: Build and maintain strong relationships with external agencies, healthcare providers, and legal resources to facilitate client referrals.
- Program Development: Participate in the evaluation and improvement of agency programs to enhance service delivery.
Qualifications
- Education: Master’s degree in Social Work (MSW) required; Licensure as a Licensed Clinical Social Worker (LCSW) is preferred.
- Experience: Minimum of 3-5 years of experience in case management, social work, or a related field.
- Communication: Excellent verbal and written communication skills with the ability to interact effectively with diverse populations.
- Empathy: Deep sense of empathy, cultural competence, and a non-judgmental approach to client interactions.
- Organization: Strong organizational skills with the ability to manage multiple caseloads efficiently.
- Computer Proficiency: Familiarity with case management software and electronic health records systems.
