Responsibilities
- Develop and implement individualized service plans for 30+ clients facing housing instability, mental health challenges, or unemployment
- Coordinate interdisciplinary teams including therapists, housing specialists, and legal advocates
- Conduct comprehensive needs assessments using evidence-based frameworks like Motivational Interviewing
- Advocate for policy changes through coalition building and testimonies at city council hearings
- Maintain HIPAA-compliant documentation while leveraging our cloud-based EHR system
- Lead crisis interventions for high-risk situations including domestic violence and substance withdrawal
- Mentor junior staff through monthly case consultations and skill-building workshops
Qualifications
- Master's degree in Social Work, Counseling, or related field (LCSW/LPC preferred)
- 5+ years direct case management experience with vulnerable adult populations
- Advanced certification in Trauma-Informed Care or Crisis Intervention
- Proficiency in data analysis tools (Excel, SPSS) for outcomes reporting
- Bilingual fluency in Spanish or Arabic (required for grant compliance)
- Valid Illinois driver's license with reliable transportation for home visits
- Experience collaborating with Chicago Public Schools and healthcare systems
- Strong understanding of HUD housing vouchers and SNAP eligibility criteria
