We are seeking a dedicated Licensed Clinical Social Worker (LCSW) to provide high-impact, empathetic support to diverse populations. You will play a pivotal role in designing treatment plans, fostering resilience, and empowering our clients to navigate complex life transitions in a supportive, multidisciplinary environment.
Responsibilities
- Conduct comprehensive psychosocial assessments to determine client needs and eligibility for programs.
- Develop and implement personalized treatment plans focused on crisis intervention and long-term stability.
- Provide individual and group counseling sessions utilizing evidence-based practices such as CBT and DBT.
- Facilitate referrals to community resources, legal aid, and medical providers to ensure holistic care.
- Maintain precise, HIPAA-compliant clinical documentation and progress notes in our integrated electronic health record system.
- Collaborate with multidisciplinary teams, including psychiatrists and case managers, to ensure cohesive care delivery.
- Advocate for client rights and accessibility to social services within the greater Chicago area.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited university.
- Current, active LCSW licensure in the State of Illinois.
- Minimum of 3 years of clinical experience in a social service or behavioral health setting.
- Deep understanding of trauma-informed care and cultural competency.
- Excellent verbal and written communication skills for effective client rapport and team coordination.
- Proficiency in digital health records software and Microsoft Office Suite.
- Ability to thrive in a fast-paced, high-need social service environment.
