Responsibilities
- Develop and execute personalized long-term care plans for youth experiencing housing instability.
- Conduct regular home visits and community assessments to monitor progress and safety.
- Facilitate cross-agency coordination with schools, mental health providers, and legal entities.
- Maintain detailed, HIPAA-compliant case documentation and reporting.
- Advocate for client resources within public assistance and private sector partnerships.
- Lead weekly support group sessions focused on life skills and conflict resolution.
- Provide crisis intervention and emergency support during and after business hours as needed.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related human services field.
- Current LSW or LCSW licensure in the State of Washington preferred.
- Minimum of 3 years of experience in case management or youth services.
- Proficiency in trauma-informed care practices and systemic social analysis.
- Strong interpersonal skills with the ability to build rapport across diverse populations.
- Valid driver’s license and reliable transportation for community travel.
- Excellent technical literacy, including proficiency in Salesforce or similar CRM tools.
