Responsibilities
- Conduct comprehensive bio-psychosocial assessments for high-needs clients.
- Develop and implement individualized service plans to meet housing, health, and employment goals.
- Facilitate interdisciplinary team meetings to coordinate holistic care delivery.
- Maintain precise, HIPAA-compliant documentation in our centralized case management database.
- Advocate for clients within the legal, medical, and governmental systems to ensure equitable access.
- Mentor junior case workers and provide clinical oversight for field operations.
- Monitor client outcomes to ensure program effectiveness and long-term sustainability.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related behavioral science field.
- Current LCSW or LSW licensure in the State of Illinois required.
- Minimum 3+ years of experience in clinical or community-based case management.
- Proficiency in trauma-informed care and crisis intervention techniques.
- Strong interpersonal skills with the ability to build rapport across diverse demographics.
- Demonstrated expertise in navigating local social service networks and public benefits programs.
- Bilingual in Spanish and English is highly preferred.
