Responsibilities
- Conduct comprehensive biopsychosocial assessments for new and existing clients to identify support needs.
- Develop, implement, and monitor individualized care plans in collaboration with clients and multidisciplinary teams.
- Provide crisis intervention and emotional support during high-stress situations or family emergencies.
- Advocate for clients' rights and navigate complex social service systems to secure housing, employment, and financial assistance.
- Maintain accurate, confidential case notes and documentation in compliance with agency standards and privacy laws.
- Facilitate support groups and workshops to foster community engagement and skill development.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (e.g., Psychology, Sociology) is required.
- Minimum of 2 years of experience in family support, case management, or social services is preferred.
- Valid Class G Driver’s License and access to a reliable vehicle for client home visits.
- Strong understanding of local community resources, government programs, and social safety nets.
- Excellent written and verbal communication skills, with the ability to build rapport quickly.
- Proficiency in Microsoft Office Suite and case management software (e.g., Salesforce, Cerner).
