Responsibilities
- Conduct Comprehensive Intakes: Perform initial assessments and comprehensive case histories to identify the unique needs and goals of each family unit.
- Develop Individualized Care Plans: Create, implement, and monitor tailored case plans that address housing stability, mental health, and family cohesion.
- Resource Navigation: Act as a primary liaison to connect clients with essential services, including healthcare, educational programs, and legal aid.
- Crisis Intervention: Provide immediate support and stabilization during high-stress situations, ensuring client safety and emotional well-being.
- Advocacy: Serve as a fierce advocate for client rights within the social services system and the broader community.
- Documentation & Reporting: Maintain accurate, confidential, and timely records in accordance with agency policies and state regulations.
Qualifications
- Education: Bachelor’s degree in Social Work (BSW), Psychology, Sociology, or a related field required.
- Experience: Minimum of 2-3 years of experience in case management, social work, or a related human services role.
- Licensure: Valid driver’s license and reliable transportation are mandatory for client visits.
- Bilingual Ability: Fluency in Spanish and English is highly preferred.
- Skills: Strong interpersonal communication skills, problem-solving abilities, and a deep sense of cultural competence.
- Attributes: Demonstrated ability to handle sensitive information with the utmost confidentiality and professionalism.
