Responsibilities
- Conduct comprehensive psychosocial assessments and intake interviews for new clients.
- Develop, implement, and monitor individualized treatment plans in collaboration with clients and their families.
- Provide individual, family, and group therapy sessions utilizing evidence-based therapeutic modalities.
- Respond to crisis situations with effective de-escalation techniques and immediate safety planning.
- Maintain accurate, timely, and confidential client records in compliance with state and federal regulations.
- Collaborate with a multidisciplinary team of healthcare professionals, educators, and community partners.
- Advocate for clients to ensure they receive necessary resources, including housing, healthcare, and financial assistance.
- Participate in ongoing training and professional development to stay current with best practices in social work.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution.
- Active Licensed Clinical Social Worker (LCSW) credential in the state of Illinois.
- Minimum of 2 years of clinical experience providing direct counseling or case management services.
- Strong understanding of crisis intervention, trauma-informed care, and family systems theory.
- Exceptional communication, active listening, and interpersonal skills.
- Ability to maintain strict confidentiality and adhere to ethical guidelines.
- Proficiency in using electronic health record (EHR) systems and case management software.
- Bilingual in English and Spanish is highly preferred but not required.
