Responsibilities
- <ul><li>Conduct thorough initial and ongoing psychosocial assessments of clients to identify needs and strengths.</li><li>Develop individualized, comprehensive care plans in collaboration with clients, families, and multidisciplinary teams.</li><li>Coordinate and monitor access to community resources, medical services, housing, and other support systems.</li><li>Provide crisis intervention and de-escalation support as needed, ensuring client safety and stability.</li><li>Maintain accurate, timely, and confidential client records in compliance with all regulatory standards.</li><li>Advocate on behalf of clients to ensure appropriate service delivery and protect their rights.</li><li>Participate in regular team meetings, supervision, and ongoing professional development.</li></ul>
Qualifications
- <ul><li>Master's degree in Social Work (MSW) from an accredited program.</li><li>Current State of Illinois Licensed Clinical Social Worker (LCSW) licensure required.</li><li>Minimum of 2-3 years of post-licensure clinical case management experience, preferably in a community-based setting.</li><li>Demonstrated knowledge of mental health diagnoses, treatment modalities, and community resources.</li><li>Exceptional communication, interpersonal, and crisis intervention skills.</li><li>Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.</li><li>Ability to work independently, manage a diverse caseload, and adapt to changing client needs.</li></ul>
