Responsibilities
- Conduct Comprehensive Assessments: Perform psychosocial evaluations to identify client needs, risks, and strengths, ensuring a foundation for personalized care.
- Develop Individualized Care Plans: Create, implement, and regularly review robust case plans that address housing, mental health, and financial stability goals.
- Crisis Intervention & Support: Provide immediate support during mental health crises or emergencies, connecting clients with urgent resources and stabilization services.
- Advocacy & Resource Navigation: Serve as a strong advocate for clients, navigating complex bureaucratic systems to secure essential benefits (SNAP, Medicaid, housing vouchers).
- Collaborative Care Coordination: Liaise with healthcare providers, schools, law enforcement, and family members to ensure a cohesive support network.
- Documentation & Compliance: Maintain accurate, timely, and confidential electronic health records (EHR) in strict adherence to HIPAA and agency policies.
Qualifications
- Education: Bachelor’s degree in Social Work (BSW) required; Master of Social Work (MSW) or LCSW/LMSW certification preferred.
- Experience: Minimum of 2-3 years of experience in social services, case management, or human services within a healthcare or non-profit setting.
- Knowledge: Deep understanding of New York City social service systems, including housing authority procedures and benefit programs.
- Communication: Excellent verbal and written communication skills, with the ability to translate complex information for diverse populations.
- Compassion: Demonstrated ability to work with diverse, underserved communities with high levels of empathy and cultural competence.
- Technical Proficiency: Comfortable using electronic case management software and Microsoft Office Suite.
- Bilingual: Bilingual in English and Spanish is a strong plus.
