  • Develop, implement, and monitor individualized case plans that align with client goals and agency objectives.
  • Advocate for clients within various systems, including healthcare, legal, and housing sectors, to secure necessary support.
  • Crisis intervention and de-escalation for clients experiencing acute mental health or substance use challenges.
  • Coordinate with external agencies, medical providers, and family members to ensure a continuum of care.
  • Maintain accurate, confidential, and timely documentation in accordance with agency and legal standards.

Qualifications

  • Master’s degree in Social Work (MSW) or Bachelor’s degree with 2+ years of direct case management experience.
  • Current licensure as an LCSW, LCSW-R, LMFT, or LMHC preferred.
  • Valid California Driver’s License and access to a reliable vehicle for client visits.
  • Bilingual skills (English/Spanish) are highly desirable.
