Responsibilities
- Conduct thorough biopsychosocial assessments to identify client needs, strengths, and barriers to stability.
- Develop and implement comprehensive, goal-oriented service plans in collaboration with clients and multidisciplinary teams.
- Maintain accurate, up-to-date electronic health records (EHR) in compliance with HIPAA and agency standards.
- Coordinate referrals to external community resources, including housing, mental health, and employment services.
- Advocate for client needs within government agencies, healthcare systems, and housing authorities.
- Facilitate regular follow-up meetings to monitor progress, adjust care strategies, and provide crisis intervention when necessary.
- Participate in weekly team case conferences to review complex cases and share best practices.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, Sociology, or a related human services field.
- Minimum of 2 years of professional experience in social work, case management, or community outreach.
- Demonstrated knowledge of local Chicago community resources and social service networks.
- Strong proficiency in documentation, reporting, and crisis intervention protocols.
- Excellent interpersonal skills with the ability to maintain professional boundaries while fostering trust.
- Valid driver’s license and access to reliable transportation for community visits.
- LSW or related licensure preferred, but not required.
