Responsibilities
- Develop and execute comprehensive individualized service plans for a caseload of 25-30 clients.
- Conduct regular home visits and field assessments to monitor progress and safety.
- Provide crisis intervention and rapid-response support for clients experiencing acute housing or financial instability.
- Maintain precise documentation in our Electronic Case Management (ECM) system in compliance with HIPAA and state regulations.
- Collaborate with local healthcare providers, government agencies, and legal counsel to advocate for client resources.
- Lead monthly group counseling sessions focused on life-skills development and social integration.
- Participate in multidisciplinary team meetings to refine program outcomes and improve service delivery models.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, Sociology, or a closely related field.
- Minimum of 2 years of professional experience in social services or community case management.
- Proven ability to manage high-stress situations with composure and professional ethics.
- In-depth knowledge of Chicago-area social support systems and local housing legislation.
- Strong interpersonal skills with the ability to build rapport with diverse, multi-cultural populations.
- Proficiency in Microsoft Office Suite and experience with cloud-based case management software.
- Valid Illinois driver’s license and reliable personal transportation.
