  • Participate in multidisciplinary team meetings to refine program outcomes and improve service delivery models.

Qualifications

  • Bachelor’s degree in Social Work (BSW), Psychology, Sociology, or a closely related field.
  • Minimum of 2 years of professional experience in social services or community case management.
  • Proven ability to manage high-stress situations with composure and professional ethics.
  • In-depth knowledge of Chicago-area social support systems and local housing legislation.
  • Strong interpersonal skills with the ability to build rapport with diverse, multi-cultural populations.
  • Proficiency in Microsoft Office Suite and experience with cloud-based case management software.
  • Valid Illinois driver’s license and reliable personal transportation.
