Responsibilities
- Conduct comprehensive biopsychosocial assessments to identify client needs and barriers.
- Develop and implement individualized service plans with measurable goals and timelines.
- Provide crisis intervention and support during emergency situations to ensure client safety.
- Coordinate referrals to external housing, mental health, and employment agencies.
- Maintain meticulous case documentation and progress notes in alignment with regulatory standards.
- Facilitate group support sessions and community outreach initiatives.
- Advocate for client needs during interdisciplinary team meetings and policy discussions.
Qualifications
- Bachelor’s Degree in Social Work, Psychology, Sociology, or a closely related field.
- Minimum of 2 years of direct experience in case management or social service delivery.
- Deep understanding of trauma-informed care and harm reduction strategies.
- Strong proficiency in electronic health record (EHR) systems and Microsoft Office.
- Excellent communication skills, with the ability to build rapport with diverse populations.
- Current LSW or equivalent professional licensure preferred.
- Valid driver’s license and reliable transportation for community site visits.
