Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans tailored to client needs.
- Provide crisis intervention and emotional support to clients facing acute life challenges.
- Link clients with appropriate community resources, including housing, employment, and healthcare services.
- Monitor client progress and adjust care plans in collaboration with interdisciplinary teams.
- Maintain accurate, confidential documentation in compliance with HIPAA and agency standards.
- Facilitate support groups and educational workshops to promote community well-being.
Qualifications
- Master’s degree in Social Work (MSW) preferred; Bachelor’s degree with relevant experience considered.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) certification required.
- Proven experience in case management, healthcare coordination, or social work.
- Strong knowledge of New York City social service systems and safety net programs.
- Excellent verbal and written communication skills with a focus on empathy and active listening.
- Ability to handle high-stress situations with resilience and professionalism.
