Responsibilities
- Conduct comprehensive assessments of client needs and develop individualized care plans.
- Coordinate and provide referrals to essential community resources, healthcare providers, and support services.
- Monitor client progress and adjust treatment plans as necessary to ensure positive outcomes.
- Maintain accurate and confidential case records in compliance with agency policies and HIPAA regulations.
- Advocate for clients in navigating complex social service systems, housing authorities, and legal processes.
- Facilitate case management meetings and provide crisis intervention support when required.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (Master’s preferred).
- Current licensure as an LCSW, LSW, or LPC (or willingness to obtain within 1 year).
- Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders.
- Strong interpersonal and communication skills with a focus on empathy and active listening.
- Ability to work independently and manage a diverse caseload efficiently.
- Proficiency in Microsoft Office Suite and case management software.
