Responsibilities
- Conduct comprehensive assessments of client needs, including mental health, housing stability, and family dynamics, to develop personalized care plans.
- Facilitate crisis intervention strategies and provide immediate support during high-stress situations to ensure client safety.
- Advocate for clients within government agencies, healthcare providers, and educational institutions to secure necessary resources and services.
- Monitor and evaluate the effectiveness of support plans, making data-driven adjustments to ensure positive outcomes.
- Maintain accurate, confidential, and up-to-date case files and documentation in compliance with regulatory standards.
- Collaborate with multidisciplinary teams, including social workers, psychologists, and legal aid, to provide holistic care.
- Organize and lead community outreach workshops focused on parenting skills, financial literacy, and healthy coping mechanisms.
Qualifications
- Master’s degree in Social Work (MSW) or a related field (Bachelor’s degree with significant relevant experience may be considered).
- Current licensure or registration with the BC College of Social Workers (BCCSW).
- Minimum of 3 years of experience in case management, social work, or community support services.
- Strong knowledge of local social service systems, housing resources, and mental health legislation in British Columbia.
- Exceptional interpersonal and communication skills, with the ability to build rapport with diverse populations.
- Proficiency in case management software and Microsoft Office Suite.
- Valid Class 5 Driver’s License and access to a reliable vehicle for client home visits.
