Responsibilities
- Develop and maintain comprehensive individualized service plans for a caseload of 25-30 families.
- Conduct thorough intake assessments to identify needs regarding housing, nutrition, and mental health.
- Coordinate referrals to local community partners, government agencies, and medical providers.
- Perform regular home visits and office-based check-ins to monitor client progress and safety.
- Document all interactions and outcomes accurately within our centralized CRM system.
- Facilitate weekly support groups and educational workshops for community members.
- Advocate on behalf of clients to secure necessary benefits and legal assistance.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, or a closely related human services field.
- Minimum of 2 years of professional experience in direct social service or case management.
- Strong knowledge of Chicago-based community resources and social safety net programs.
- Demonstrated ability to maintain professional boundaries and handle confidential information.
- Excellent communication skills with the ability to build rapport with diverse populations.
- Proficiency in Microsoft Office Suite and electronic case management software.
- Valid driver’s license and access to reliable transportation for community outreach.
