Responsibilities
- Develop and implement individualized service plans for families in crisis.
- Conduct thorough biopsychosocial assessments to identify resource gaps and needs.
- Coordinate referrals to local healthcare, housing, and educational service providers.
- Maintain precise, confidential case records in compliance with state and federal regulations.
- Facilitate weekly advocacy sessions to support family goal achievement and self-sufficiency.
- Participate in multidisciplinary team meetings to review case progress and outcomes.
- Conduct home visits and field outreach to ensure accessibility and safety.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, or a related human services field.
- Minimum of 2 years of professional experience in social services or case management.
- Deep understanding of trauma-informed care principles and intervention strategies.
- Excellent interpersonal skills with the ability to build trust in diverse communities.
- Proficiency in case management software and Microsoft Office Suite.
- Valid driver’s license and reliable personal transportation.
