Responsibilities
- Conduct comprehensive intake assessments to identify family needs and set achievable goals.
- Develop and implement individualized service plans with regular progress monitoring.
- Facilitate direct referrals to housing, employment, and mental health resources.
- Maintain accurate, confidential electronic case records in compliance with agency standards.
- Coordinate multi-disciplinary team meetings to ensure holistic support for clients.
- Advocate on behalf of clients to remove systemic barriers to essential services.
- Provide crisis intervention and stabilization support during high-pressure situations.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, or a related human services field.
- Minimum of 2 years of experience in case management or direct client advocacy.
- Deep understanding of trauma-informed care and person-centered service delivery.
- Strong written and verbal communication skills for documentation and client interaction.
- Proficiency in case management software and Microsoft Office Suite.
- Ability to navigate complex social service systems within the Chicago area.
- Valid driver’s license and reliable transportation preferred.
