Responsibilities
- Conduct thorough biopsychosocial assessments to identify client needs and risk factors.
- Develop, implement, and monitor individualized care plans in collaboration with clients and stakeholders.
- Advocate for clients to ensure access to healthcare, housing, and community resources.
- Coordinate with medical providers, legal aid, and family members to support holistic care.
- Maintain accurate, confidential, and up-to-date case documentation and electronic health records.
- Provide crisis intervention and emotional support during high-stress situations.
- Facilitate support groups and educational workshops to promote community resilience.
Qualifications
- Master’s degree in Social Work (MSW) or a related field.
- Valid State Licensure (LCSW, LMSW, or LMFT) required.
- Minimum of 2-3 years of experience in case management or social services.
- Strong knowledge of local social service systems and community resources.
- Excellent written and verbal communication skills.
- Proficiency in Microsoft Office Suite and case management software (e.g., Salesforce, Cerner).
- Ability to handle sensitive information with the highest level of confidentiality.
