Key Responsibilities:
- Conduct thorough intakes and assessments to understand client needs, history, and goals.
- Develop, implement, and monitor individualized case plans that promote stability and independence.
- Connect clients with essential resources, including housing, healthcare, employment services, and financial assistance.
- Advocate for clients within various social service systems and government agencies.
- Maintain accurate and confidential case records using our database management systems.
- Crisis intervention and emotional support for clients facing acute challenges.
Qualifications:
- Bachelor’s degree in Social Work (BSW), Psychology, or a related field (or equivalent combination of experience and education).
- Valid Driver’s License and reliable transportation.
- Proven experience in case management or social services (1-3 years preferred).
- Strong interpersonal and communication skills, with the ability to build rapport quickly.
- Deep empathy, cultural competence, and a non-judgmental approach.
- Ability to manage a heavy caseload while meeting strict deadlines.
- Proficiency in Microsoft Office Suite and case management software.
