Responsibilities
- Conduct comprehensive client intakes and initial assessments to determine service needs and eligibility.
- Develop and implement individualized case plans that address the physical, emotional, and social needs of clients.
- Coordinate with community partners, government agencies, and healthcare providers to secure necessary resources.
- Monitor client progress and adjust service plans as necessary to ensure successful outcomes.
- Advocate effectively on behalf of clients to navigate complex social service systems.
- Document all client interactions and service delivery in compliance with agency and regulatory standards.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (Master’s degree preferred).
- Active State Licensure (LMSW, LCSW, or LSW) required.
- Minimum of 3 years of experience in social services or case management.
- Strong knowledge of local, state, and federal social service regulations and policies.
- Excellent verbal and written communication skills.
- Demonstrated ability to work independently and manage a high volume of cases.
