Responsibilities
- Conduct comprehensive initial assessments and ongoing evaluations of clients' physical, emotional, and social needs.
- Develop, implement, and monitor individualized care plans in collaboration with multidisciplinary teams and service users.
- Provide crisis intervention and emotional support to clients facing acute mental health or family crises.
- Advocate for clients to ensure they receive appropriate social, medical, and financial support services.
- Maintain accurate and up-to-date case records, ensuring full compliance with legal and regulatory standards.
- Facilitate group workshops and community outreach programs to promote mental health awareness.
- Supervise junior social work students and trainees when applicable.
Qualifications
- Hold a recognized qualification in Social Work (CQSW, BA, MA, or equivalent) and be fully registered with the Health and Care Professions Council (HCPC).
- Demonstrate a minimum of 3-5 years of post-qualification experience in adult or mental health social work.
- Strong understanding of relevant UK legislation, including the Care Act 2014 and Mental Health Act 1983.
- Excellent communication and interpersonal skills, with the ability to build rapport quickly and effectively.
- Proficiency in case management software (e.g., Cygnet, Carefirst) and Microsoft Office Suite.
- Ability to work under pressure and manage a diverse caseload with a high degree of professionalism.
