Responsibilities
- Conduct comprehensive assessments of children and families' needs, ensuring safety and welfare are prioritized.
- Develop, implement, and review individualized care plans in collaboration with service users and multidisciplinary teams.
- Act as a key worker, providing direct support, advocacy, and guidance to vulnerable individuals.
- Monitor cases closely, identifying risks and escalating issues to safeguard children effectively.
- Liaise with external agencies, including health professionals, educational settings, and legal bodies.
- Maintain accurate, up-to-date case records in line with statutory requirements and agency policies.
Qualifications
- Valid Social Work qualification (CQSW/QSW) and registration with the Health and Care Professions Council (HCPC).
- Minimum of 3 years post-qualified experience in a statutory or similar social care setting.
- Deep understanding of relevant legislation, including the Children Act 1989 and the Care Act 2014.
- Strong assessment and risk management skills.
- Excellent communication and interpersonal abilities, with the capacity to build rapport quickly.
