Responsibilities
- Conduct Comprehensive Assessments: Perform thorough evaluations to understand the physical, emotional, and financial needs of each client, ensuring personalized care plans are developed.
- Develop and Implement Care Plans: Create actionable, measurable goals with clients to support housing stability, employment, and mental wellness.
- Resource Navigation: Identify and connect clients with critical services, including healthcare providers, legal aid, and financial assistance programs.
- Case Documentation: Maintain accurate, up-to-date case files and detailed documentation in accordance with agency policies and HIPAA regulations.
- Client Advocacy: Act as a strong voice for clients in meetings with government agencies, healthcare providers, and court systems.
- Crisis Intervention: Provide immediate support and guidance during high-stress situations to ensure client safety and well-being.
- Collaborative Teamwork: Work closely with a multidisciplinary team to ensure holistic support for the community.
Qualifications
- Education: Bachelor’s degree in Social Work (BSW) or a related field required; Master’s degree (MSW) preferred.
- Licensure: Current LCSW or LMSW license is highly preferred.
- Experience: Minimum of 3-5 years of experience in case management, social work, or a related human services field.
- Communication Skills: Exceptional verbal and written communication skills with the ability to interact effectively with diverse populations.
- Problem Solving: Strong ability to troubleshoot challenges and navigate complex bureaucratic systems.
- Computer Proficiency: Must be proficient in Microsoft Office Suite and case management software.
- Driver's License: Valid driver’s license and reliable transportation required for client outreach.
