Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans tailored to client needs.
- Provide direct counseling and crisis intervention services to adults and families facing mental health challenges or housing instability.
- Collaborate with healthcare providers, legal aid, and government agencies to secure essential resources and referrals.
- Monitor client progress and adjust treatment plans as necessary to ensure positive outcomes.
- Maintain accurate, confidential case notes and documentation in compliance with HIPAA and agency standards.
- Facilitate support groups and educational workshops to promote community resilience.
Qualifications
- Master’s degree in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) preferred.
- Current valid New York State Social Work license.
- Minimum of 3-5 years of experience in case management or social services, preferably in a healthcare or community setting.
- Strong knowledge of community resources, mental health laws, and advocacy strategies.
- Exceptional interpersonal communication skills and cultural competency.
- Ability to work independently and manage a high caseload effectively.
