Responsibilities
- Develop and implement individualized intervention plans for clients facing housing instability, mental health challenges, and systemic barriers
- Conduct comprehensive psychosocial assessments using evidence-based frameworks
- Coordinate multidisciplinary care teams including psychiatrists, legal advocates, and housing specialists
- Facilitate therapeutic groups focused on trauma recovery and life skills development
- Advocate for systemic policy changes through community partnerships and research initiatives
- Maintain accurate electronic case records while ensuring HIPAA-compliant data management
- Supervise junior staff and social work interns through formal mentorship programs
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 3 years post-MSW experience in direct client services
- Registered with the Ontario College of Social Workers (RSW)
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Expertise in harm reduction approaches and crisis intervention protocols
- Advanced proficiency in case management software (e.g., Cvent, Salesforce Nonprofit Cloud)
- Demonstrated success in securing external funding through grant applications
- Valid Ontario driver's license with access to reliable transportation
