Responsibilities
- Develop and implement comprehensive case management plans for clients facing complex psychosocial barriers
- Provide crisis intervention, trauma-informed counseling, and advocacy for vulnerable populations including refugees, LGBTQ+ youth, and individuals experiencing homelessness
- Collaborate with healthcare providers, legal services, and community organizations to create integrated support networks
- Conduct psychosocial assessments and produce detailed documentation for court reports and funding applications
- Mentor junior staff and supervise practicum students while maintaining rigorous quality standards
- Lead community outreach initiatives to identify at-risk populations and reduce service barriers
- Analyze program outcomes using data-driven methods to enhance service delivery models
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 5 years of progressive experience in direct social services with complex case management
- Registered member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Proven expertise in trauma-informed care, crisis intervention, and cultural competency
- Demonstrated success in collaborating across sectors (healthcare, legal, education)
- Advanced knowledge of relevant legislation including Child, Youth and Family Services Act and Ontario Human Rights Code
- Strong data analysis skills with experience in outcome measurement and program evaluation
- Valid Ontario Driver's License and access to reliable transportation for community visits
