Responsibilities
- Develop and implement comprehensive case management plans for vulnerable populations
- Provide crisis intervention and trauma-informed counseling to individuals and families
- Collaborate with healthcare providers, schools, and government agencies to coordinate services
- Conduct community outreach to identify at-risk populations and connect them with resources
- Advocate for policy changes addressing systemic barriers in social services
- Maintain detailed case documentation and prepare reports for funding agencies
- Mentor junior social workers and interns on best practices
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care
- Experience developing culturally responsive programs for diverse populations
- Strong knowledge of Chicago's social services ecosystem and resources
- Excellent documentation skills with experience in electronic health records
- Valid driver's license and reliable transportation for home visits
