Responsibilities
- Conduct Comprehensive Assessments: Perform thorough psychosocial evaluations to determine the needs of clients and develop appropriate care plans.
- Crisis Intervention: Provide immediate, high-quality crisis counseling and intervention for individuals facing acute mental health or social challenges.
- Case Management: Coordinate with healthcare providers, legal aid, and housing agencies to ensure holistic support for clients.
- Advocacy: Serve as a strong advocate for client rights, navigating complex systems to secure necessary resources and services.
- Documentation: Maintain accurate, confidential, and timely electronic health records (EHR) in compliance with HIPAA regulations.
- Supervision: Mentor junior social workers and interns, providing guidance on clinical techniques and case management strategies.
Qualifications
- Licensure: Valid New York State LCSW (Licensed Clinical Social Worker) or LMSW (Licensed Master Social Worker) license required.
- Experience: Minimum of 3-5 years of clinical experience in a social services or healthcare setting.
- Education: Master’s degree in Social Work (MSW) from an accredited institution.
- Communication: Exceptional written and verbal communication skills with the ability to engage diverse populations.
- Bilingual: Proficiency in English and Spanish is highly preferred.
- Technology: Proficient in using EMR/EHR systems (e.g., Epic, Cerner) and Microsoft Office Suite.
