Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized service plans for clients.
- Advocate for clients within government agencies, healthcare facilities, and legal systems.
- Monitor client progress and adjust treatment plans to ensure optimal outcomes.
- Crisis intervention and emergency response coordination.
- Manage a caseload of 20-30 clients, ensuring timely documentation and compliance.
- Collaborate with multidisciplinary teams to provide holistic care.
Qualifications
- Master's degree in Social Work (MSW) preferred; Bachelor's degree with 5+ years of experience may be considered.
- Valid New York State Social Work license (LMSW or LCSW).
- Strong understanding of social welfare policies and community resources.
- Excellent written and verbal communication skills.
- Proficiency in Microsoft Office Suite and case management software.
- Demonstrated ability to handle sensitive information with the highest level of confidentiality.
