Responsibilities
- Develop and implement comprehensive care plans for clients facing housing instability, mental health challenges, and economic hardship
- Coordinate with healthcare providers, legal advocates, and government agencies to create integrated support networks
- Conduct biopsychosocial assessments using evidence-based tools to identify client needs and barriers
- Provide crisis intervention and ongoing counseling using trauma-informed methodologies
- Maintain detailed case documentation while ensuring HIPAA compliance and confidentiality
- Lead community outreach initiatives to identify at-risk populations and expand service accessibility
- Mentor junior staff through clinical supervision and professional development workshops
Qualifications
- Master's degree in Social Work, Counseling Psychology, or related field (LCSW/LMSW preferred)
- Minimum 5 years' experience in direct case management with vulnerable populations
- Proven expertise in crisis intervention and trauma-informed care practices
- Strong knowledge of Chicago's social services ecosystem and resource networks
- Exceptional cultural competency with diverse populations including LGBTQ+ and immigrant communities
- Certification in Mental Health First Aid or equivalent crisis response training
- Advanced proficiency in electronic health records (EHR) systems and data analytics
- Valid Illinois driver's license with reliable transportation for home visits
