Responsibilities
- Assess patient health needs, develop, and implement comprehensive care plans in collaboration with physicians and the interdisciplinary team.
- Administer medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
- Monitor patients' vital signs and medical conditions, promptly identifying any changes that require immediate intervention.
- Educate patients and their families on post-discharge care, medications, and lifestyle modifications.
- Document patient care activities, charting accurately and thoroughly in the Electronic Health Record (EHR).
- Collaborate with nursing staff and other healthcare professionals to ensure a seamless continuum of care.
- Participate in quality improvement initiatives and adhere to all hospital policies and regulatory standards.
Qualifications
- Current, valid Registered Nurse (RN) license in New York State.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 2 years of experience in a critical care or telemetry setting preferred.
- Strong critical thinking, decision-making, and communication skills.
- Ability to work effectively in a fast-paced, high-stress environment.
