Responsibilities
- Provide evidence-based individual, family, and group therapy using modalities like CBT, DBT, and trauma-focused approaches
- Conduct comprehensive psychosocial assessments and develop personalized treatment plans
- Coordinate care with multidisciplinary teams including psychiatrists, nurses, and community partners
- Facilitate crisis intervention and safety planning for high-risk clients
- Lead outreach programs to connect vulnerable populations with essential resources
- Maintain accurate clinical documentation compliant with HIPAA and state regulations
- Mentor junior staff and contribute to program quality improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Current Illinois LCSW license with at least 3 years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care practices
- Experience working with diverse populations including LGBTQ+ communities and refugees
- Strong assessment, diagnostic, and treatment planning abilities
- Excellent communication skills with demonstrated cultural competency
- Familiarity with electronic health records (EHR) systems
- Valid driver's license and reliable transportation for home visits
