Responsibilities
- Conduct thorough initial client assessments to identify social, emotional, and physical needs.
- Develop, implement, and monitor individualized service plans (ISPs) tailored to each client’s goals.
- Crisis intervention and emotional support during high-stress situations.
- Coordinate with healthcare providers, legal aid, housing agencies, and educational institutions.
- Maintain accurate, confidential, and up-to-date case documentation in accordance with regulatory standards.
- Advocate for client rights and ensure access to essential community resources.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (Master’s preferred).
- 1-3 years of experience in social services, case management, or community outreach.
- Valid New York State Driver’s License and access to reliable transportation.
- Deep understanding of local community resources and social service systems.
- Strong computer proficiency with case management software (e.g., Salesforce, CareLog).
- Exceptional interpersonal and communication skills.
