Responsibilities
- Conduct comprehensive initial assessments and develop individualized care plans tailored to client needs.
- Provide crisis intervention and emotional support during high-stress situations.
- Facilitate connections to external resources, including housing assistance, employment services, and medical providers.
- Monitor client progress and adjust care plans as necessary to ensure positive outcomes.
- Maintain accurate and confidential electronic health records in compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams to ensure a cohesive support network for clients.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (Master’s preferred).
- Valid New York State Social Work Licensure (LMSW or LCSW) or eligibility to obtain.
- Minimum of 2 years of experience in case management or social services.
- Strong interpersonal and communication skills with the ability to build rapport quickly.
- Proficiency in Microsoft Office Suite and case management software.
- Ability to manage a caseload of 20+ clients effectively.
